Home finger-prick kits are convenient and the NHS does not use them for testosterone. Here is what actually differs, why squeezing the finger changes the number, and when to confirm a result properly.

Why squeezing your finger changes the number

This is the specific failure that matters and it is worth understanding, because it is the reason finger-prick results come back odd rather than simply wrong.

Capillary blood only flows freely for a short time. When the drops slow, people squeeze — and squeezing does two things. It ruptures red blood cells (haemolysis), and it forces interstitial fluid from the surrounding tissue into the sample, diluting it. Both distort results, and labs routinely reject samples for haemolysis.

Add the postal step. The sample sits in a padded envelope for a day or more, possibly in a warm van, before it is analysed. Testosterone is reasonably stable, but the combination of a small squeezed sample and a delay is why these tests carry more variability than a venous draw taken and spun the same morning.

If you use a finger-prick test, do these five things

  • Take it before 11am, ideally between 7am and 10am. Timing matters more than the collection method — see morning vs evening testing.
  • Warm your hand thoroughly first. Run it under warm water and let the arm hang down. Good flow is the single best defence against having to squeeze.
  • Do not squeeze the fingertip. Milk gently from the base of the finger if you must, and wipe away the first drop.
  • Post it the same day, early enough for that day's collection, and avoid posting on a Friday or before a bank holiday.
  • Confirm anything low or borderline with a venous test before drawing conclusions or buying anything.

What a venous test lets you measure that a finger prick often cannot

Sample volume is the constraint. A full assessment of low testosterone is not one number, and the extra markers are frequently what explains a confusing result.

If you only ever measure total testosterone, a borderline result tells you very little. That is the real argument for a venous panel, more than accuracy per se.
